Meloxicam l\u00e0 m\u1ed9t d\u1eabn xu\u1ea5t oxicam c\u00f3 t\u00e1c d\u1ee5ng h\u1ea1 s\u1ed1t, gi\u1ea3m \u0111au v\u00e0 ch\u1ed1ng vi\u00eam. Meloxicam ho\u1ea1t \u0111\u1ed9ng b\u1eb1ng c\u00e1ch l\u00e0m gi\u1ea3m t\u1ed5ng h\u1ee3p prostaglandin (li\u00ean quan \u0111\u1ebfn qu\u00e1 tr\u00ecnh vi\u00eam, s\u1ed1t, \u0111au) do \u1ee9c ch\u1ebf cyclooxygenase (COX).<\/p>\n Meloxicam \u1ee9c ch\u1ebf \u01b0u ti\u00ean tr\u00ean COX-2. Li\u1ec1u d\u00f9ng v\u00e0 s\u1ef1 kh\u00e1c nhau gi\u1eefa c\u00e1c c\u00e1 th\u1ec3 ng\u01b0\u1eddi b\u1ec7nh \u1ea3nh h\u01b0\u1edfng \u0111\u1ebfn m\u1ee9c \u0111\u1ed9 \u1ee9c ch\u1ebf COX-1. Li\u1ec1u 15 mg\/ ng\u00e0y \u1ee9c ch\u1ebf COX-1 nhi\u1ec1u h\u01a1n li\u1ec1u 7,5 mg\/ ng\u00e0y. So v\u1edbi c\u00e1c thu\u1ed1c \u1ee9c ch\u1ebf kh\u00f4ng ch\u1ecdn l\u1ecdc COX, Meloxicam \u00edt c\u00f3 t\u00e1c d\u1ee5ng ph\u1ee5 tr\u00ean ti\u00eau h\u00f3a h\u01a1n.<\/p>\n Meloxicam h\u1ea5p thu t\u1ed1t v\u00e0 k\u00e9o d\u00e0i qua \u0111\u01b0\u1eddng ti\u00eau h\u00f3a v\u1edbi sinh kh\u1ea3 d\u1ee5ng tuy\u1ec7t \u0111\u1ed1i kho\u1ea3ng 89%. Sau khi u\u1ed1ng v\u00e0o l\u00fac \u0111\u00f3i, n\u1ed3ng \u0111\u1ed9 t\u1ed1i \u0111a trung b\u00ecnh \u0111\u1ea1t \u0111\u01b0\u1ee3c l\u00e0 4 – 5. V\u00e0o ng\u00e0y th\u1ee9 5 khi u\u1ed1ng li\u1ec1u l\u1eb7p l\u1ea1i, thu\u1ed1c \u0111\u1ea1t tr\u1ea1ng th\u00e1i \u1ed5n \u0111\u1ecbnh<\/p>\n S\u1ef1 h\u1ea5p thu c\u1ee7a thu\u1ed1c \u00edt b\u1ecb \u1ea3nh h\u01b0\u1edfng b\u1edfi th\u1ee9c \u0103n. Cmax t\u0103ng kho\u1ea3ng 22%, Tmax t\u1eeb 5 – 6 gi\u1edd, AUC kh\u00f4ng thay \u0111\u1ed5i khi u\u1ed1ng vi\u00ean nang meloxicam sau b\u1eefa \u0103n s\u00e1ng b\u00e9o. Cmax v\u00e0 AUC kh\u00f4ng \u0111\u1ed5i, Tmax t\u0103ng l\u00ean t\u1edbi kho\u1ea3ng 7 gi\u1edd khi u\u1ed1ng d\u1ea1ng dung d\u1ecbch. Kh\u00f4ng c\u00f3 t\u01b0\u01a1ng t\u00e1c d\u01b0\u1ee3c \u0111\u1ed9ng h\u1ecdc v\u1edbi antacid.<\/p>\n Th\u1ec3 t\u00edch ph\u00e2n b\u1ed1 trung b\u00ecnh kho\u1ea3ng 10 L. Kho\u1ea3ng 99,4% thu\u1ed1c g\u1eafn v\u1edbi protein huy\u1ebft t\u01b0\u01a1ng (ch\u1ee7 y\u1ebfu l\u00e0 albumin). T\u1ef7 l\u1ec7 g\u1eafn k\u1ebft protein kh\u00f4ng ph\u1ee5 thu\u1ed9c v\u00e0o n\u1ed3ng \u0111\u1ed9 v\u00e0 gi\u1ea3m xu\u1ed1ng kho\u1ea3ng 99% \u1edf b\u1ec7nh nh\u00e2n b\u1ecb b\u1ec7nh th\u1eadn. Sau khi u\u1ed1ng, meloxicam ph\u00e2n b\u1ed1 v\u00e0o h\u1ed3ng c\u1ea7u ng\u01b0\u1eddi v\u1edbi n\u1ed3ng \u0111\u1ed9 < 10%, > 90% li\u1ec1u ph\u00f3ng x\u1ea1 d\u01b0\u1edbi d\u1ea1ng kh\u00f4ng thay \u0111\u1ed5i \u0111\u01b0\u1ee3c ph\u00e1t hi\u1ec7n trong huy\u1ebft t\u01b0\u01a1ng.<\/p>\n N\u1ed3ng \u0111\u1ed9 meloxicam trong d\u1ecbch kh\u1edbp sau khi d\u00f9ng li\u1ec1u \u0111\u01a1n kho\u1ea3ng 40 – 50% n\u1ed3ng \u0111\u1ed9 trong huy\u1ebft t\u01b0\u01a1ng. T\u1ef7 l\u1ec7 d\u1ea1ng t\u1ef1 do trong huy\u1ebft t\u01b0\u01a1ng th\u1ea5p h\u01a1n 2,5 l\u1ea7n so v\u1edbi trong d\u1ecbch kh\u1edbp.<\/p>\n Meloxicam chuy\u1ec3n m\u1ea1nh \u1edf gan d\u01b0\u1edbi s\u1ef1 x\u00fac t\u00e1c ch\u1ee7 y\u1ebfu c\u1ee7a CYP2C9 v\u00e0 m\u1ed9t ph\u1ea7n nh\u1ecf c\u1ee7a CYP3A4 th\u00e0nh c\u00e1c ch\u1ea5t chuy\u1ec3n h\u00f3a 5\u2019-carboxymeloxicam (60% li\u1ec1u), \u0111\u01b0\u1ee3c t\u1ea1o ra b\u1edfi s\u1ef1 oxy h\u00f3a 5\u2019-hydroxymethylmeloxicam, ch\u1ea5t n\u00e0y c\u0169ng th\u1ea3i tr\u1eeb \u1edf m\u1ed9t m\u1ee9c \u0111\u1ed9 th\u1ea5p h\u01a1n (9% li\u1ec1u). Ho\u1ea1t t\u00ednh c\u1ee7a peroxidase c\u1ee7a 2 ch\u1ea5t chuy\u1ec3n h\u00f3a c\u00f2n l\u1ea1i chi\u1ebfm 16% v\u00e0 4% li\u1ec1u d\u00f9ng t\u01b0\u01a1ng \u1ee9ng. C\u00e1c chuy\u1ec3n h\u00f3a \u0111\u1ec1u kh\u00f4ng c\u00f3 t\u00e1c d\u1ee5ng d\u01b0\u1ee3c l\u00fd.<\/p>\n Meloxicam th\u1ea3i tr\u1eeb v\u1edbi l\u01b0\u1ee3ng nh\u01b0 nhau qua ph\u00e2n v\u00e0 n\u01b0\u1edbc ti\u1ec3u, ch\u1ee7 y\u1ebfu d\u01b0\u1edbi d\u1ea1ng ch\u1ea5t chuy\u1ec3n h\u00f3a,. T\u1ef7 l\u1ec7 thu\u1ed1c ch\u01b0a chuy\u1ec3n h\u00f3a trong n\u01b0\u1edbc ti\u1ec3u v\u00e0 ph\u00e2n v\u00e0 l\u1ea7n l\u01b0\u1ee3t l\u00e0 0,2% v\u00e0 1,6%.
